AcademyHealth Releases a New Report on Reducing Hospital Readmissions

Reducing avoidable hospital readmissions represents a unique opportunity for policymakers, payers, and providers to reduce health care costs while improving the quality of care. In January 2008, AcademyHealth conducted a meeting, sponsored by the Commonwealth Fund, to discuss hospital readmission trends, identify best practices for preventing readmissions, and examine policy levers that could be used to reduce readmissions. The resulting report, authored by HCFO staff, synthesizes the main themes of discussion and presents best practices and potential policy levers for reducing hospital readmissions.
